Parental consent form for school trips – template
Before a trip or excursion, schools usually collect written consent from parents. Below is a template you can print and fill in by hand. In EduTrip the same form is filled in from the trip details automatically. The printable form itself is in Czech, as used by Czech schools.
What the consent form should include
- The organiser – school and accompanying teacher
- Name, date, departure point and destination
- Price per person, if the event is paid
- Child's name, class and date of birth
- Allergies and health restrictions
- Parent or guardian contact
- Information on personal data processing
- Date and signature of the parent or guardian

Já, níže podepsaný/á zákonný zástupce, souhlasím s účastí svého dítěte na výše uvedené školní akci a beru na vědomí pravidla, která stanovil pedagogický doprovod. Pro případ akutních zdravotních potíží opravňuji pedagogický doprovod k zajištění nezbytné lékařské pomoci.
Zpracování osobních údajů
Osobní údaje žáka uvedené v tomto souhlasu zpracovává škola jako správce pouze pro účely organizace této akce. Údaje nebudou předány třetím stranám mimo přepravce a destinaci akce, kteří je potřebují k provedení služby. Údaje budou smazány do 30 dnů po skončení akce.
